What is Live Well Suffolk? A free healthy lifestyle service, commissioned by NHS Suffolk A 3-4 year service Operates on two levels: – Universal service for the whole County (except Waveney) – Targeted service, with more intensive interventions where most needed

Service components Universal service Includes specialist service and supporting GP practices & pharmacies in delivering Level2 advice Smoking cessation Targeted service (20% most deprived MSOAs) Includes Health Champion volunteer scheme Community Health coaches Targeted service (20% most deprived MSOAs + Criteria) Adult weight management Universal service (Within Criteria) Child weight management Universal service Includes marketing campaigns, community events, resource distribution and training Healthy Lifestyle Messaging

Access criteria -Adult weight management: BMI of >30 or >28 with a co-morbidity -Child weight management: >91 st percentile -CHC 1:1 : no BMI or weight criteria but live in an MSOA or AC -CHC groups: no BMI/weight criteria but live in an MSOA or AC -Smoking cessation: universal

Specialist Stop Smoking We offer intensive one to one and group support for those wishing to stop smoking Advice for specialist groups such as pregnancy and mental health etc Pharmacotherapy on prescription Support for those advisors working within primary care and pharmacies Training for brief intervention and Level 2 advisors Annual Updates for smoking cessation advisors

Adult Weight Management

Lose Weight With Live Well Suffolk 12 week specialist programme Week 1 and week 12 – 1:1 interview Weeks 2 – 11 group intervention Topics include: Psychological eating habits Physical activity Crash dieting All group sessions include some sort of physical activity 5% Weight Loss

Key Messages Sustained weight lose (no quick fixes) 5% weight loss Exercise v Food Practical tips and sessions to help with weight loss such as cook and eat session + store tour Active minutes Screen time

Lose Weight with Live Well Suffolk for Kids Specialist Child Weight Management programme working closely with the percentile charts 1:1 Interview with parent and child Group1:1 Fortnightly sessions over 16 weeks + 2 follow up sessions to complete 6 months of support 5 sessions over a period of 6 months Parent and child together (learning and PA sessions) Parent and child together (Learning sessions) Works closely with the Lose Weight with LWS for kids specialist booklet Monthly follow ups after intervention

Community Health Coach Team

What do we do? Live Well Suffolk Community Health Coaches can offer expert support and advice on a wide variety of health related issues. Our clinics and courses are targeted to the areas most at need in regards to health intervention Our work includes- Helping clients to stop smoking through 1 to 1 or group provision Supporting clients through a 1 to 1, 6 session personal health plan. These can address a wide array of issues from basic weight management, improving diet and increasing physical activity. Guiding clients through 6 week physical activity or cook and eat courses with bolted on personal health plans Signposting on to other relevant services and opportunities
