Supporting Individual Success This is the name of a big project to make changes in Maine about your services. What’s that name again? Supporting Individual Success!

What does that mean? Individual Success Supporting

Purpose of Today’s Session The State wants to share information with you about Supporting Individual Success and also hear what you have to say.

What the State is trying to do & why The State wants: You to get the services & supports you need to live in the community. This also includes people waiting for services!

Services You Get The state wants to make sure that the services and money people get are: Fair for everybody Based on your needs and choices Available when you need them

It’s okay that along the way, we may not always agree! What & How Now we will talk about: What we need to do, and How we will do it. It’s okay that along the way, we may not always agree!

Person- centered system that can last over time We Need to Balance What’s Important to people What’s Important to the system Person- centered system that can last over time

Moving Forward To move forward and reach our goals, we must: Build on the successes we’ve achieved, Be willing to change some things Balance our wants with the best use of the money we have.

How Will it Work? Find out about your need for support. Get a SIS Get a Level Get a Budget Get a Plan Get Services Find out about your need for support. Low Support Needs High

What is the Supports Intensity Scale? The SIS measures a person’s support needs. This is done during an interview.

The interview measures your support needs… What support do you need? by asking you and other people who know you well a set of questions. 13

How Will it Work? Find out about your need for support. Based on your support needs, you will get a level. 1 2 3 4 5 Low Support Needs High

How Will it Work? Find out about your need for support. Based on your support needs, you will get a level. You get a budget you can spend to get your services. 1 2 3 4 5 Low Support Needs High Low Money High

About your Actual Budget Your Budget for services is made up of 2 parts: A budget is how much money you have and how you plan to spend the money. Where you live What you do during the day. Other services like therapies, career planning, employment specialist services, and assistive technology are added on to your base budget if you need them.

+ Base Budgets = 1 2 3 4 5 Budget SIS Level Where You live Day Activities + = Budget

Review What’s the project name? What’s it do? If you could ask any question, what would you ask the person in charge?

What Do I Need to Do? Think about what a good life means for you. Think about what you enjoy doing. Think about the possibility of getting paid to do something you enjoy. Get out and give back. There are many ways to be a part of your community.

What Do I Need to Do? Speak up. If something doesn’t seem right about your supports, tell someone. Make choices that are right for you. Think about the life you want to live and choose the supports you need. Having freedom and flexibility comes with responsibility. Make sure you are prepared. Make sure that your service plan gets you the supports you need.

What else is important for me to know right now? No changes have been made to your services at this time. Information about levels is for review only. Based on review and public comments, changes may be made to what has been proposed. Change process will begin next year, starting in July 2015. Information is posted on the OADS website: http://www.maine.gov/dhhs/oads/disability/ds/sis/ OADS wants YOUR feedback & comments! Email OADS@maine.gov
