Joinder Agreement… What’s That? This is the legally binding document that you need to eventually fill out and have signed and notarized It contains info about the Trust you need to know (and) It contains spaces for you to fill in your own info The Arc of Washington State 10-1-2003

(p.3) Primary Donor & Beneficiary This is the person who is setting up, filling out the documents, and submitting the trust fund application. This is the person with the developmental disability for whom the trust is being established. The Arc of Washington State 10-1-2003
(p.4) Legal Representative & Primary Representative This is the person who is a legal representative for the beneficiary. (This could be a guardian or rep. payee. Also, if the beneficiary is under 18 the parents are the legal guardians) This is the person who will request disbursements and receive the quarterly statements for the account. (p.7) Insurance Policies and Disability Health and Burial Policies Disability Describe the beneficiary’s developmental disability. Is the beneficiary covered under any health insurance policy, or prepaid burial insurance plan? The Arc of Washington State 10-1-2003
