Recommended Exams Medical, Dental and Eye exams are encouraged (routine check-ups) Forms included in packets

Health Alerts Health history (obtained from parent/physician input) updated yearly and “as needed” or requested Shared with school staff on a “need to know” basis Determine if it is a Health alert or IHP (individualized health plan) IHP developed between school nurse and parent –Indicates health condition/illness –Specifies school implications and or emergency measures needed –Identifies parent contact information (important to keep current)

Medications School Policy Medications must be brought to school in original labeled container with student name Medication authorizations –Prescription medication require parent and provider signature –Over the counter medications require parent signature –Forms available in school office and district website
