The Invoice Invoice must have a number Submitted on Practice headed notepaper Include name of F2 Supervisor Include name of F2 Trainee Date from / Date to Amount claimed Payee and Bank Account details Please post the invoice directly to the address in Wakefield

Submitting the invoice Invoices should be submitted during the last month that you have the trainee in post with you Deadline for invoice submission – 2 months after completion of F2 trainee supervision period. Retrospective payments will be considered. Foundation School approves payments via Oracle system

Payment Enquiries Attach copy of invoice, confirming name of F2 trainee, date of invoice and payee details Payee details will be searched on the Oracle payments system
