Children who will be five (5) years old by September 30, 2012 are eligible for Kindergarten. Eastwood School’s Kindergarten Registration for the 2012 – 2013 school year will begin March 1, Eligible families may pick up the registration packet from the school office between 9:30 a.m. and 3:00 p.m. Monday through Friday. In addition to the registration packet, please note the following requirements for registration: Original Birth Certificate, Original Social Security Card, Oath of Residency Statement, Guardianship/Custody papers when applicable, current Physical and Proof of Immunizations with exact dates. Original documents (birth certificate, etc.) will be photocopied and returned to you. All parts of the registration packet must be completed and turned in together to the office. Your child will not be registered until all documents have been received. Class lists will be available after August 13 th at Eastwood Elementary. All kindergarten families are asked to register by April 30 th in order for your child to participate in Kindergarten Screening scheduled for May 25, Please sign up in the office for your child’s Kindergarten Screening appointment when you pick up your registration packet.

STEP 1: COMPLETE SCHOOL REGISTRATION PACKET Student Enrollment packet (pink) Oath of Residency Statement – “Proofs” are not accepted until after August 1 st at the Oberlin City SD Administrative 153 North Main Street, bring photo ID at time of submission Emergency Medical Authorization/Field Trip Consent form (green) Ohio Dept of Health – Health History form (green) Physician Assessment form – completed by physician Free/Reduced meals application Additional forms may be required at the start of school RETURN PACKET WITH ATTACHMENTS & SIGN UP FOR MAY 25TH SCREENING PRIOR TO APRIL 30TH

STEP 2: PARENT & CHILD ATTEND SCREENING DAY Screening will take place on Friday, May 25, 2012 at Eastwood Elementary School. Students will not be screened until all required paperwork is submitted. On your screening day you will be greeted by school professionals. Students will be escorted to various stations and parents will attend a briefing by the Principal. A school nurse will evaluate general health: vision, speech and hearing. Learning-related functions and school-readiness assessments will be done at various stations. This is not a pass-fail screening. Please plan to spend about one hour completing the screening process.

STEP 3: PROVE RESIDENCY Starting August 1st please bring a minimum of 3 current items (within 30 days of August 1st) along with the Oath of Residency Statement that was included in your registration packet to: Board of Education Office 153 North Main Street, Oberlin Present with a photo ID of the parent/legal guardian If you cannot locate the form you may download one from the following link

All new kindergarten students and their families are invited to our Kindergarten Sneak Peek Spaghetti Dinner April 19, 5:30 P.M. It will be a chance to meet other new kindergarten students and their families as well. You can meet the kindergarten teachers and get a chance to see the kindergarten classrooms. If you have a child whose birthday falls between October 1 st and December 31 st of this year and you are interested in having your child assessed for early entrance into kindergarten, you must indicate your interest in writing to Eastwood’s Principal before May 1, Part I of testing for early entrance will be on May 25, 2012, during Kindergarten screening at Eastwood School. Pending the results of Kindergarten screening, the Pupil Service office will contact you to schedule an appointment for Part II of the Early Entrance Assessment.

Eastwood Elementary School is closed from June 25 through August 12, 2012 Classroom assignments will be available after August 14, 2012 by calling Eastwood Elementary at Student fees and classroom supply lists will be posted on the web site in July

TRANSPORTATION Please call the Transportation Department at after August 1 st and they will be happy to assist you with your child’s transportation needs. HOW DO I KNOW IF MY CHILD IS READY FOR KINDERGARTEN? There isn't just one indicator that determines whether your child is ready for kindergarten. Experts agree that a child's development needs to be evaluated in several areas. Some school districts use assessment tests to determine kindergarten readiness. Children are asked questions to test their cognitive abilities. They might also be asked to perform tasks such as drawing shapes and sorting objects. Experts advise parents not to make a decision based entirely on test results, but to consider observations by teachers, pediatricians and parents.

HOW DO I KNOW IF MY CHILD IS READY FOR KINDERGARTEN? The following is a guideline that includes a range of social, academic and developmental factors to consider when deciding if your child is ready to enter school: Enthusiasm toward learning. Is he eager to explore and discover? Is he comfortable asking questions? Does he persist even when a task is difficult? Language skills. Does she communicate her needs? Express her feelings appropriately? Ability to listen. Can he follow simple instructions? Is he able to listen to an entire story without interrupting? Desire to be independent. Does she separate from parents for the school day? Is she starting to take responsibility for her personal belongings? Can she follow simple two-step tasks? Can she use the bathroom by herself? Ability to interact with children and adults. Is he able to share, compromise, take turns and problem-solve? Strong fine-motor skills. Is she able to hold and use a pencil? Cut with scissors? Is she learning to write her name? Basic letter and number awareness. Can he sing and recite the alphabet and recognize some letters? Can he count to 10 and identify numbers one to five?
