nd & 3 rd Quarter.  In the 2 nd and 3 rd quarter of 2011, TNMC recorded 2 Lost Time Injuries (LTIs). The 2 nd LTI was also a reportable injury. 5 dangerous occurrences were recorded. There were no RWI reported while 3 MTI’s and 2 FAI’s were reported.  At the end of September LTIFR was 0.11 with a severity rate of 1.59.

3 Significant Incidents Highlights (LTIs)  On 7 th April 2011 at around 14:00hrs Ofentse Monikaso. a semiskilled fitter and colleagues were aligning agitator motor at DMS module 1. The motor slipped from the chain block and hit Ofentse on the right leg resulting in a laceration and muscle contusion.  On 20th September 2011 at around 0830hrs, Mr. Seabelo Sekwati - a semi- skilled boilermaker - and his colleagues were turning a chute to lay it flat on the workshop floor. In the process, Seabelo's right index finger got trapped between the chute base and the floor, and there was an avulsion of the finger tip up to the first digit.

5 Environment  2 incidents were recorded during the last 2 quarters. The first incident was a slime spillage while the second incident was a water spillage which resulted in water flowing outside mine lease area. The latter incident was reported as a dangerous occurrence.  TNMC is currently developing a Mine Closure plan

6 TNMC SHEQms  TNMC continues to maintain its SHEQms.  An internal audit was conducted in July as planned. Areas of concern included document control and poor house keeping.  A surveillance audit was conducted by DQS in September. 16 findings were raised during this audit. A follow up audit will be conducted in December.  Pinagare campaign activities focused on Sustaining TNMC through resilience, External stakeholder engagement, Adherence and compliance.
