What is the role of CDE, K-12 HSN and State Library?  Have no statutory authority to administer the federal E- rate program  May only offer general information about the E-rate/CTF program including: training and outreach, reference materials, and other publicly available SLD/USAC resources  The following information is provided to assist applicants with their applications. It remains each applicant’s responsibility to ensure FCC/USAC, State, and local agency rules and practices are followed. All liability and risk rests upon each applicant for E-rate approval. Form 471 filing window: – Opens at noon EST on Wednesday, January 14, 2015 – Closes at 11:59 PM EST on April 16, 2015 Last possible day to file the Form 470: – Thursday, March 20, 2015 Deadlines E-rate Modernization

What is the Form 471? – The mechanism to request funding Specific services, e.g. voice, digital transmission Specific dollar amount Specific locations Specific time frame Certifies that you followed all program rules Form 471 E-rate Modernization

When to File the Form 471 E-rate Modernization Posted for 28 days (including RFP if using) Received bids 470 AFTER 28 days – evaluated bids Filled out Bid Evaluation Matrix Bids Awarded bids (Board Approval if needed) Signed contracts Award April 16, 2015 at 8:59 p.m. BEFORE

A new Form 471 must be filed EVERY YEAR – For Month-to-Month services – For Contracted services Have key information ready prior to starting your Form 471 – Enrollment and NSLP (use upload template to assist in collecting data) – Category 2 budget by site (if requesting Cat 2 services) – Details for Online Item 21 (templates will help you know what information is needed) Form Reminders E-rate Modernization

New online Form 471 – Designed to separate Category 1 services from Category 2 services – Category 1 Phase Out services must be on its own 471 Plan ahead to identify cost for services being phased out and put them on a separate application from Category 1 services that are not being phased out.
