Two Distinct Applications You must meet the requirements for BOTH of the following: 1) San Jose State University 2) School of Nursing The School of Nursing has different application deadlines.

University Guidelines Enroll the first semester you are admitted! Pay fees on time or DROPPED from classes Pay attention to Add and Drop official deadlines Late drops are ONLY for SEVERE and compelling reasons (work itself is not an adequate excuse; if medical, documentation is required) Check your class schedule every two weeks to make sure you have not been dropped

Mandatory University Orientation All Freshmen must attend the mandatory FROSH orientation All transfer students must attend the Transfer Orientation Program (TOP)

“Undeclared” or Change Of Major Students University does not allow changing majors if you have accumulated more than 90 units Submit Change of Major forms to the University Advising Department (NOT Nursing Office) Visit the Academic Advising and Retention Services website at www.sjsu.edu/advising Located at Student Services Center (10th Street and San Fernando Street)

Students with 90 Units or Greater NOTE: Those students nearing or have greater than 90 units CANNOT apply to Nursing without formal written permission from the AVP of Student Success Center, Dr. Maureen Scharberg. SJSU Writing Skills Test (WST) Sign up at least 3 months prior to application Allow at least 4 weeks to receive results SJSU Testing Department can answer all WST questions SJSU Testing Department located at Industrial Studies Industrial Studies Building 228

WST Exam Dates 2015 Refer to the SJSU Testing website at http://testing.sjsu.edu/ for more information The testing fee of $38.00 and the late registration fee of $15.00 are non-refundable. Fees are subject to change.

Test of Essential Academic Skills (TEAS) MUST take Version V with a minimum score of 75% Higher score from the first 2 times taken will be used Results will be available on the ATI website at www.atitesting.com after 7-10 days Attach results to the School of Nursing Application

Submitting Your Application

When To Submit Do not submit the application until COMPLETE with all documentation, including sealed transcripts All prerequisites must be completed

Impaction

Scoring for Impaction See page 2 of BS information packet for details (TEAS Score divided by 23) + GPA for 5 prerequisites GPA for 5 prerequisites must be at least a 3.0 The selection of SJSU nursing students for each cohort is solely based on the students’ impaction scores Side Note: The cutoff score for Spring 2014 was 7.19 [must still meet supplemental criteria of 3.0, complete a background check, and get a drug clearance]. The score for Fall 2014 pool was 7.09 initial cut.

Letters of Provisional Acceptance

Acceptance Notification Acceptance or non-acceptance letters within 10-11 weeks of Nursing Application Deadline Submission date OPEN Provisional Acceptance Letters immediately as there are response deadlines New student nursing workshop fee is non- refundable Orientation workshop is Mandatory

“Provisional Acceptance” Must do the following AFTER receiving Provisional Acceptance: Submit a Health Statement (MD or NP exam) Drug Screening Background Check Health Insurance

SJSU Studies

SJSU Studies You do not have to do these prior to applying to the Nursing major MUST be taken at SJSU Used to be called “Advanced General Education” or “Upper Division” courses Must have at least 60 units at SJSU and pass the WST or get a waiver signed to take SJSU Studies courses When lower division classes are done at a Community College, best to obtain a “General Education certification” Refer to SJSU Class Schedules at info.sjsu.edu for more information

4 Areas of SJSU Studies Completed in order to graduate, but not in order to apply to the Nursing major Must take 1 course in each of these areas as a graduation requirement: Area R: Earth and Environment Area S: Self, Society, and Equality in the U.S. NURS 138 in 6th semester in Nursing Area V: Culture, Civilization, and Global Understanding Area Z: Writing Communication II Only specific courses allowed

Area Z of SJSU Studies Must be completed by the end of the first year of Nursing program HPROF 100W or NURS 100W Other approved options: RECL 100W, BIOL 100W, PSYC 100W, KIN 100W, HRTM 100W, CHAD 100W

SJSU Studies Exemption Second Baccalaureate students who earned their first Bachelors degree from a regionally accredited institution in the United States: SJSU Studies Area R and Area V are exempted MUST still take NURS 138 and Area Z (HPROF 100W)

SJSU Studies Timeline What happens if I get admitted to the university, but still do not have all prerequisite courses or nursing requirements completed (i.e. TEAS)? Answer: You are NOT eligible to apply to Nursing, but you may start taking your SJSU Studies courses if you have passed the WST exam
