*Disposition of Students  Did the student return to class?  Go home? Parent request? Nurse?  Was the student transported by EMS?  Was the student sent to hospital, SBHC or health care provider’s office by other means?  Other / Specify such as police or CYFD 26

Remember:  The total number of student visits = the total number in the disposition section.  Only count the student in one category of each visit and list disposition each time. 27
