Sports Physicals MSHSAA Physical Forms have changed to include a more comprehensive exam that includes signing a waiver that states that you and your parents have read the new concussion packet regarding the dangers of concussions. The concussion packets are linked to our school website. *Completed physicals include all four pages (this can be accessed on the LSN web site): medical check-up, health record, insurance, concussion review, parent and student signatures.
