7 Transition Timeline Date of Birth May 15, 2012 Notice to CPSE of potential eligibility by: September 1, 2014 Transition Conference convened by: October 1, 2012 Date child first potentially eligibility for CPSE services January 2, 2015 Last date for referral to CPSE to ensure eligibility determination before age three February 14, 2015 Evaluation conducted: Between September 1, 2014 and before 3 rd Birthday (allowing time for meeting to be conducted) CPSE Meeting Prior to May 15, 2015 Last date for a child found eligible for CPSE services to receive EIP services August 31, 2015 Recommendations forwarded to BoardAfter meeting (recommendations)
