F1 – IEP - Health Related Direct Service activities: Providing health/mental health services contained in an IEP. Medical/health assessment and evaluation as part of the development of an IEP Conducting medical/health assessments/evaluations and diagnostic testing and preparing related reports Providing speech, occupational, physical and other therapies Performing developmental assessments Providing counseling services to treat health, mental health, or substance abuse conditions Transportation (if covered as a medical service under Medicaid)

F2 – Non IEP - Direct Service activities: Providing personal aide services Administering first aid, or prescribed injections or medications to a student Providing direct clinical/treatment services Developing a treatment plan (medical plan of care) for a student if provided as a medical service Performing routine or mandated child health screens including buy not limited to vision, hearing, dental, scoliosis, and EPSDT screens Providing immunizations

F2 – Non IEP - Direct Service activities: School staff should use this code when making referrals for, coordinating, and/or monitoring the delivery of medical (Medicaid covered) services. Referral, coordination, and monitoring activities related to services in an IEP are reported in this code.

P Code School staff should use this code when making referrals for, coordinating, and/or monitoring the delivery of medical (Medicaid covered) services. Referral, coordination and monitoring activities related to services in an IEP are reported in this code. Activities that are part of a direct service are coded F.

Examples of P Code 1.Participate in meeting/discussion to coordinate or review a student’s needs for health-related services covered by Medicaid. 2.Provide information to other staff on the child’s medical/dental/mental health services and plans. 3.Monitor and evaluate the Medicaid service components of the IEP as appropriate. 4.Make referrals for and/or coordinate medical or physical examinations and necessary medical/dental/mental health evaluations. 5.Gather any information that may be required in advance of referrals. 6.Provide follow-up contact to ensure has received the prescribed medical/dental/mental health services covered by Medicaid. 7.Make referrals for and/or scheduling EPSDT (CHEC) screens, interperiodic screens, and appropriate immunizations, but NOT to include state-mandated health services 8.Refer students for necessary health, mental health, or substance abuse services covered by Medicaid.

19.5 Hour or Voucher Employees * Only submit codes for the hours paid by Uintah School District * Voucher Employees should not use the “Q” code

Contract Employees * Only submit codes for the hours on contract 35 hour employees – should not use “Q” for after contract hours. * If a contract employee takes PTO time, the PTO hours will be marked as “Q”
