 Provide brag sheet Complete brag sheet to provide information to your grader Provide examples and be quantitative if possible  Provide Dream Sheet  Be familiar with the FITREP form IOT provide the appropriate information Include if you are not in standards Provide swim qual/sail qual information

(DDMMMYYY ie 10NOV1775 )  This block will be completed as one of the following: Block 6: regular will be checked Block 7: date will be in same format at block 5  Marines will have number format (###)  Navy will have one of the following: SM SH GL GM GH EL EM EH OL OM OH Or F for failure, P for probationary, M for medical waiver  Individual being graded will provide graders information if not within standards Block 8 and 9: information to be provided by individual being graded (MON YEAR)

Block 10:  a: NROTC billets or MIR  b: major (NOT in all capitals) or undeclared Block 11: NROTC awards listed first, then university honors. If none, then write N/A Block 13: enter name of active duty advisor in all capitals in RANK LASTNAME format Ex: MAJ LANCASTER, LT BURICH, etc

Block 21: BLANK Block 22: primary graders make billet recommendation for next semester. If no billet is recommended, complete with MIR Blocks 23-24: graders enter comments in upper and lower case format. Do not include the name of the person being graded multiple times. Comments will be in 10pt Times New Roman font. Graders name will be entered in all caps in the following format: MIDN X/C FI. MI. LASTNAME Ex. MIDN 1/C T. J. WARD

 Comments should reflect individual accurately and be positive  Quantifiable is best, but quality is important too  Describe the person, don’t use general comments Great example of a midshipmen. Shows potential.  How? Clippard has stepped up to act as platoon sergeant several times this semester and has taken on the additional responsibilities of this position when needed. He has shown great potential for this billet in the future. He also pushes himself at every PT session and aspires to one day be able to run as fast as Ward.

Block 25: enter the name of the individual’s active duty advisor in RANK LASTNAME format  Ex: MAJ LANCASTER, LT BURICH, etc  All other blocks are left blank
