Our staff are trained and skilled at reviewing a claim file, identifying issues that warrant a referral to SIU, setting action plans, and conducting investigations that support and defend claims decisions. Serving as the SIU designate for your company, our staff will file the referral to the State of Florida DFS - Division of Insurance Fraud and NICB (for member companies). They will also file the Fraud Plan with DIF and conduct Fraud Awareness Training for all claims staff.

 $ 625 Flat Rate $ 85 each additional hour requested.  A verbal and report of the results will follow the operation within 24 hours.  The investigative report will be provided within 3 days of the completion of the assignment via . The hard copy report with any video documentation on CD will be provided within seven days.

$ 650 Flat Rate – COMPREHENSIVE (schedule, notification letter, prep, conduct, report, or no show) * This Flat Rate applies to EUO assignments that do not involve field investigation activity. Rate is for file review and preparation, EUO notification, and conducting EUO.

 Clinic Inspections:  Flat Rate $ 550  or $ per hour  Clinic Inspections include a site visit by the Investigator with photos of the facility exterior and interior (interior when permitted).  The Investigator will compile a list of all equipment used and connected to billing procedures. The Investigator will also report on all licensed and non-licensed personnel at the facility and a licensure check of staff. Inspection will also include a database search and court records check of all identified owners or corporate officers.

 Background Checks: $ 35 Flat Rate National Comprehensive Report + Associates: Includes Summary Report, Phones Plus, Bankruptcy, Liens/Judgments, UCC Filings, Fictitious Business Name, People at Work, Driver’s Licenses, Vehicle Investigations, Motor Vehicle Gateway, Property, Watercraft, FAA Pilots, FAA Aircraft, Professional Licenses, Florida Accidents, Voter Registration, Hunting / Fishing Permits, Concealed Weapons Permits, Federal Firearms & Explosives, DEA Controlled Substances, Criminal Records, Sexual Offenders, Associates, Relatives (3 Degrees) and Neighbors.

There is a difference in the service we provide! We are not Private Investigators trying to conduct SIU and claims investigations. We are claims professionals, licensed adjusters, with SIU experience, licensed as Private Investigators conducting claims and SIU investigations. Call or us.
