Searching by Provider type… You can search by one of the following provider types: Frontline/Industrial Medical Clinics Ambulatory Surgery Centers General Hospitals Other Facilities & Services Primary Treating Physicians Other Physicians & Healthcare professionals Select a provider type from the drop down menu

Creating a provider list - search by location or provider name To produce a list of providers within a specific mileage enter the address and # of miles from location you wish to search To create a list of providers, Click on the ‘Find Provider Now’ button Note: if you wish to ‘drill down’ to within a specialty to find a provider by language, or board certification, select ‘further refine search’.

Viewing your online directory Online directory features: Click on the ‘provider name’ to drill down into provider information to check for board certification, accepting new patients, etc. Click on the map symbol to view and print map and driving directions.
