17 HP & Medicaid 1.Office purchases a vial of 17 HP from a compounding pharmacy 2.Administer a dose to the patient 3.Office bills MA under special program and is reimbursed maximum $20/dose Important Caveats: 1.The pharmacy MUST use 17HP from Medisca Inc. or Gallipot Inc. 2.There was a problem with the Gallipot NDC numbers, and only the one ending in “04” may actually work. 3.The medication is for office use only. ***You will not find pharmacies willing to accept MA for 17HP because pharmacies cannot participate directly in the program and are paid less.

Selecting A Pharmacy Is the pharmacy PCAB accredited? Does the pharmacy routinely test their compounded preparations? Is the pharmacy familiar with the 17HP Medicaid program requirements? Is the pharmacy USP compliant? What does the facility "look" like? Do they have a cleanroom and clean air equipment for sterile compounding? Is the pharmacist available for consultation? Do the personnel appear competent from observation and questions asked? Is the pharmacy "clean" and "well-organized"? Does the pharmacy have any unresolved complaints filed with the state board of pharmacy?

17 HP Important Facts Storage: Room Temperature-the vials have lots of preservative. Packaging: Should be in amber vial, requires protection from light. Properly prepared 17HP should be ready to use, no mixing, no filtering, etc. Crystallization occasionally can occur. If it does, gentle warming should re-dissolve, discard if it does not.
