EVIDENCE BASED DISEASE PROGRAM  Traditionally, PATH programs have targeted older adults with one or more chronic illness (COPD, Diabetes, heart disease, arthritis, strokes, hypertension, etc.).  PATH programs can be great resources for caregivers and those under 65 as well. Anyone who wants to take part is welcome!  Diabetes PATH workshops are very similar to PATH with a specific focus on preventing low blood sugar and other serious complications of diabetes, eating healthier, more-balanced meals, and managing symptoms that may accompany diabetes.  Chronic pain PATH is designed to teach skills needed to improve health and manage symptoms of pain.  Good candidates may have: chronic musculoskeletal pain (neck, shoulder, back, etc.), fibromyalgia, repetitive strain injuries / sports injuries, post- surgical pain (lasting more than 3 months), or migraines. Helping Medicare beneficiaries transition back into the community following a hospital admission. Working with a Community Health Worker who is certified in the Coleman Care Transitions™ model, patients identify their health risks to help avoid a re-hospitalization.
