Living a Healthier Lifestyle Jenna is a 26 year old female who wanted to change her life

Jenna’s History Weight: 143Height: 5’1”BMI: 28  ACL reconstruction in 2003, still feeling pain  Smokes 1 pack cigarettes a day  Eats take-out and microwave dinners daily  CREATION Assessment showed weakness in nutrition and trust

Patients Goals Short Term: To Begin Active Lifestyle  Get Gym membership and workout regularly  Fitness test in the beginning, every 2 months Cardio Testing Strength Testing  CREATION: “…activity is key to living His prescription for full life” (Cummings, 2008). Intermediate: To Eat Healthier  Consult with Nutritionist on healthy diet  Maintain a Diet log Monthly weight Monthly Taping of waist, thigh, calf, arms  CREATION: “Good nutrition is the process of balancing God’s great gift for full health”(Cummings, 2008). Long Term: To Quit Smoking  START Technique – Set date, Tell Someone, Anticipate, Remove, Talk to the doctor  Observe the amount of cigarettes she smokes daily Smoking Journal with cravings, feelings and amount per day to show trend  CREATION: Reducing her amount of cigarettes will allow her to increase activity

How is Jenna Doing? The most important thing is NOT how much weight Jenna has lost or how many inches… The most important thing would be how she feels:  Jenna has stated this month during her transition: “I can lift so much more weight than I could a month ago “3 people have asked me if I have lost weight!” “I am wearing a bikini, I haven’t worn one in years!!!”

Conclusion By completing the CREATION assessment, I was able to focus on Jenna’s spiritual needs and also her physical needs. By addressing Jenna’s physical needs, I was able to also connect with her psychological needs. She has improved dramatically in the past month in mind, body and spirit, and has decided to make her changes a permanent lifestyle change.
