ASO-8-1 Safety Excellence Is Global Mission Success

ASO-8-2 Safety Excellence Is Global Mission Success  Operational Hazard Defined:  Any condition, action, or set of circumstances compromising the safety of Army aircraft, associated personnel, property or equipment

ASO-8-3 Safety Excellence Is Global Mission Success  Correcting Hazards:  Operational hazards should be corrected at the lowest level possible  On the spot corrections vs. OHR (lowest level)

ASO-8-4 Safety Excellence Is Global Mission Success  Operational Hazards may involve:  Air Traffic Control (ATC)  Airways and NAVAIDs  Controller procedures/techniques  Near mid air collisions  Aircraft operations

ASO-8-5 Safety Excellence Is Global Mission Success  Aircraft maintenance or inspections  Weather services  Airfield facilities and services  Flight or maintenance training  DOD, FAA, & ICAO regulations, directives, and publications

ASO-8-7 Safety Excellence Is Global Mission Success  DA Form 2696-R fulfills the NATO STANAG 3750FS "Airmiss Reporting and Investigation"  DA Form 2696-R compliments the FAA’s Aviation Safety Reporting System (ASRS)

ASO-8-9 Safety Excellence Is Global Mission Success  OHRs will not be used:  To report flight violations for punitive actions  To determine misconduct for line of duty investigations  By evaluation boards (FEB, etc.) to determine pecuniary liability  As evidence for disciplinary action

ASO-8-10 Safety Excellence Is Global Mission Success  RM process and OHRs  ID the Hazard - originator/ASO  Asses the Hazard - ASO  Develop CM, make Risk Decision - ASO recommends, block 11  Implement Controls - commander lists corrective actions, block 12  Supervise/Evaluate - all satisfied?

ASO-8-11 Safety Excellence Is Global Mission Success  Submitting OHRs:  Anyone may submit an OHR  Anonymity of originator is acceptable but precludes a response  OHR is not required when an accident or deficiency report is completed

ASO-8-12 Safety Excellence Is Global Mission Success  When airborne report to ATC and follow up with a written report  DA Form 4755 (para 2-3b)

ASO-8-13 Safety Excellence Is Global Mission Success  Administering the Program  Filled out OHRs in triplicate Original - USASC, if applicable Original - USASC, if applicable First copy to originator First copy to originator Second copy to unit archives Second copy to unit archives  Be legible, in ink, typed or computer printed

ASO-8-14 Safety Excellence Is Global Mission Success  Routing of an OHR  When forwarding data, request investigation results, including corrective actions, be returned to the sender  Correspondence will state OHR information to be used for accident prevention only

ASO-8-15 Safety Excellence Is Global Mission Success  OHR will be submitted to the appropriate ASO or Flight Operations  If filed through Flight Operations, they will forward the OHR to the ASO  Forward OHRs pertaining to other units to the ASO of that unit

ASO-8-16 Safety Excellence Is Global Mission Success  Forward OHRs to USASC (Attn: CSSC-SA) for: DA Staff, Major Army Commands (MACOMs), or units not in the chain of command DA Staff, Major Army Commands (MACOMs), or units not in the chain of command Other Department of Defense (DOD) services or a host nation Other Department of Defense (DOD) services or a host nation

ASO-8-18 Safety Excellence Is Global Mission Success  Provide OHRs submitted on civil aviation operations to FAA Flight Standards District Office (FSDO) (see fig 2 -1)  Aviation hazards noted at Air Force or Navy installations should be reported to the base operations office

ASO-8-19 Safety Excellence Is Global Mission Success  OHRs on Army Air Traffic Control (ATC) forwarded through the MACOM to the Director, US Army ATC Activity  Near collisions, air or ground, will be reported to the FAA Office of Aviation Safety

ASO-8-25 Safety Excellence Is Global Mission Success  ASOs will:  Manage the unit OHR program  Promptly investigate. In block 11, (investigation, recommendations), the ASO should seek to identify system defects, then recommend appropriate countermeasures.
