LEAP and GEE Summer Retest Materials Delivery Materials will arrive June Large-print and braille materials will arrive June 19. Additional materials request deadline is June 20. The Student Test Coordinator must submit requests for additional materials to the District Test Coordinator no later than the morning of June 20.

Testing Site Packing List Located in the box with a green label: “Packing list enclosed” Contains information for the school to use for verification and location of materials For the summer retest, materials for schools preassigned to a testing site are packed together NOTE: Due to packing procedures, security ranges may NOT be consecutive from one pack to the next.

Testing Site Security Checklist Checklist includes a summary and a detailed list of secure materials assigned to a testing site. Keep checklists on file at school and district for one year. Do not return to DRC. There is a blank page at the end of the checklist to record security numbers of additional materials.

Sample GEE Student/Label Roster These columns indicate the subjects in which students are required to retest. For students who do not receive preidentified labels, record the number from the matching pairs of linking or single-document labels affixed to their answer documents.

Sample LEAP Student/Label Roster LEAP students will receive a preidentified label for English Language Arts and/or Mathematics. For students who do not receive preidentified labels, record the number from the matching pair of linking labels or the single- document labels affixed to their answer documents.

Applying Bar-Code Labels to Answer Documents * with the exception of nonpublic school and approved home study program students who did not receive preidentified labels; the DTC will apply labels to these students’ answer documents. Answer documents that are to be scored should have a label here. *

Preidentified Student Bar-Code Labels For the summer test only, preidentified labels should be affixed to answer documents only for students who are expected to test. Unused bar-code labels must be returned with pickup 2 materials. GEE The District Test Coordinator will distribute all preidentified labels to summer testing sites before testing sessions. All students who have been identified for summer remediation will receive a preidentified label. This includes students who took the GEE February Seniors Only Retest. If a student took no part of a test and did not attend makeup testing, return the student’s unused answer document with all other unused answer documents for the school. (The unused answer document may have a preidentified, linking, or single-document label attached.) LEAP

Linking Labels If a LEAP or GEE student taking tests on two answer documents does not receive a set of preidentified labels, the test administrator must affix a matching pair of linking labels to the student’s answer documents. If a GEE student does not receive any preidentified labels and is taking tests on three answer documents (Math, ELA, and Sci/Soc), the test administrator must affix a matching pair of linking labels to two of the answer documents and a single-document label to the third answer document. All three answer documents must be hand-coded identically according to instructions. NOTE: The District Test Coordinator will affix linking or single-document labels to answer documents for approved home study program and LEAP nonpublic school students after testing. See pages in the Test Coordinator Manual.

Single-Document Labels For each student who does not receive a preidentified label and is required to test using only a single answer document, the test administrator must affix a single-document label to the answer document. The test administrator must hand-code the answer document according to directions in the Test Administration Manual.

Possible Label Combinations For GEE only: If a student must test using three answer documents and does not receive preidentified labels, a set of linking labels and a single-document label must be used to link the three answer documents. All three answer documents must be hand-coded according to instructions for each type of label; the hand-coded student information must match exactly. For LEAP and GEE: If a student receives a single preidentified label but needs to test using two answer documents, the test administrator must affix the preidentified label to one of the student’s answer documents and a single-document label to the other answer document. The answer document with the single-document label must be hand-coded to match the information on the preidentified label exactly. For GEE only: If a student receives two preidentified labels but needs to test using three answer documents, the test administrator must affix the preidentified labels to the Mathematics and English Language Arts answer documents and a single-document label to the Science/Social Studies answer document. The answer document with the single-document label must be hand-coded to match the information on the preidentified label exactly. + PreID Single-Doc PreID + Single-Doc + Linking

Approved Home Study Program and LEAP Nonpublic School Student Labels The DTC will distribute to appropriate summer testing sites preidentified labels for approved home study program and LEAP nonpublic school students. If approved home study program or LEAP nonpublic school students do not receive preidentified labels, the test administrator must hand-code the answer documents but must not affix labels from the summer testing site to these answer documents. The DTC will affix linking or single-document labels to these answer documents after testing. The STC must return all answer documents for approved home study program and LEAP nonpublic school students at the top of box 1 of pickup 2 materials. The DTC must remove these answer documents from school boxes, label any answer documents returned without preidentified labels (using the linking and single-document labels provided), and return them with pickup 2 district materials in the appropriate envelope provided. See pages and 27 in the Test Coordinators Manual for returning test materials for approved home study program and LEAP nonpublic school students in pickup 2.

Unscorable Document Labels UNSCORABLE DOCUMENT LABEL (BLUE) ^ ALIGN TOP OF LABEL ^ THE DISTRICT TEST COORDINATOR MUST FOLLOW THE SPECIAL INSTRUCTIONS PROVIDED WITH THIS LABEL. D510509NOTSCORE DOCUMENTATION REGARDING USE OF THIS LABEL MUST BE KEPT ON FILE AT THE DISTRICT OFFICE FOR ONE YEAR. IIIIIIIIIIIIIIIIII BAR CODE IIIIIIIIIIIIIIIIIIII INSTRUCTION SHEET (PROVIDED TO DTC WITH SHEET OF UNSCORABLE DOCUMENT LABELS) Unscorable document labels must be used when an answer document is torn or soiled or when a student tested on an answer document with a preidentified label for another student. The test administrator must contact the School Test Coordinator (STC) immediately if an answer document should be designated unscorable. The STC then must contact the District Test Coordinator (DTC) immediately. The DTC will provide instructions for transferring student responses to a scorable answer document. The new scorable answer document must be returned by the school with all other scorable answer documents. The damaged answer document must not be thrown away! The DTC will affix a blue Unscorable Document label to the damaged answer document and return it with the unused district overage answer documents in pickup 2. Documentation regarding the incident must be kept on file at the district office for one year.

Summer Testing Site–Code Labels (for overage answer documents) Before distributing a district overage answer document (or an answer document requested from DRC as additional test material) to a particular testing site, the District Test Coordinator must affix a summer testing site–code label to the back page. Paper bands and paper-band labels identify groups of answer documents for processing. Paper-band labels are affixed by the School Test Coordinator. Pointers on Packing Materials for Return Use Security Checklists to track materials. Ensure that test administrators sort and label answer documents according to instructions. Keep classroom stacks of used answer documents together with paper bands. Return answer documents for approved home study program students and LEAP nonpublic school students to the District Test Coordinator in pickup 2. (If these answer documents do not have preidentified labels, the DTC must affix labels after testing.) Use paper-band labels. (School Test Coordinator affixes these.) Sort materials according to packing charts for each pickup provided in the Test Administration Manual and the Test Coordinators Manual. Ensure that all test materials returned in pickup 3 are nonscorable. Scorable materials must be kept separate.
