Nashville Promise Neighborhood Comprehensive Asthma Management Program

According to the latest information available from the American Lung Association, the Centers for Disease Control and Prevention (CDC), and the National Institute of Allergy and Infectious Diseases (NIAID): Approximately 6.8 million children in the US have been diagnosed with asthma, one of the leading, serious, chronic illness among children in the US. Asthma accounts for 13 million absences from school each year. Asthma is the third-ranking cause of childhood hospitalizations under the age of 15. Why is Asthma a NPN Issue?

NPN Comprehensive Asthma Management Program utilizes the implementation of a multidimensional approach incorporating evidence based practices from Boston Children’s Hospital Community Asthma Initiative and The Children’s Hospital of Philadelphia Community Asthma Prevention Program. NPN’s program will consist of peer to peer support, in school time asthma education, and home visitation based on 3 levels of intervention. It includes nurse case management and care coordination combined with home visits Goal- educate families about asthma, assess the home for asthma triggers, and provide materials and services to improve the home environment, such as encasements for bedding, HEPA vacuums and pest control. What is our approach?

Interventions Based on Level of Need Level 1: Assessment of home environment and instructions on abating asthma triggers. Referral to community class. Follow up visit 6 months post enrollment with review of asthma management Total of 2 visits Level 2: Assessment of home environment Instruction of environmental triggers class Provide supplies for environmental intervention: mattress and pillow covers, shades, bucket, rags, cleaner, storage bins, roach bait, etc. Refer to community classes Follow up visit at 6 months with review of asthma management Total of 3 visits Level 3: Assessment of home environment Series of 4 classes in the home Provide supplies for environmental intervention: mattress and pillow covers, shades, bucket, rags, cleaner, storage bins, roach bait, etc. Monthly follow up and data collection for 6 months Total of 6 visits and monthly follow-ups

Community Asthma Initiative As of March 2012, 900 patients have enrolled. So far their involvement has lead to: 80% reduction in the percent of patients with any asthma related hospitalization 60% reduction in the percent of patients with any Emergency Department visits 42% reduction in the percent of patients with any missed school days for children 47% reduction in the percent of patients with any lost work days for parents
