What We Do Clinical Commissioning Groups are led by GPs. We commission: Hospital Services Community services such as district nursing or physiotherapists Mental Health services Learning Disability Services
Our Vision To improve patient experience by working in partnership with patients and the local population of south Manchester and providing care close to home through the integration of health and social care services. Improving health and increasing life expectancy by promoting and enabling self care, preventing ill health and providing fair and equitable access to local health and social care services.
What We Believe We believe care should be arranged around the needs of the individual patient We believe local people should be involved in the planning and design of local services We will work to prevent ill health, to create a self-reliant, resilient and healthy population
Involving People Patient and Public Advisory Group GPs and “What keeps you healthy” session with the Young Peoples Parliament with South Manchester People First The Palatine Contraception and Sexual Health Service is a community universal service: an accessible template will be completed for learning disability / adult. The Big Health Check Up Event – was a great success. Lots of people attended, had health checks and told us about what changes needed to happen. The most important was that services were accessable.
Our Plans We want to know about the health of learning disabled people. We need to get the best information. We will continue to work with Manchester City Council (Living Longer, Living Better). Make sure that people only go to hospital if they need to and stay only as long as needed (Winterbourne) We want GP’s to know how to keep learning disabled people healthy and safe SMCCG are making sure that learning disabled people are ‘flagged’ when people go into hospital. This year we are developing a GP champion to make sure that these things happen
