Who is eligible? Must meet all criteria below:  Permanent or indefinite disability  Are currently not hospital patients  Hold one of the following cards: – Pensioner Concession Card; or – Commonwealth Seniors Card; or – Health Care Card; or – receive Carer’s Payment; or – can demonstrate financial hardship.  Are in a residential situation for independent living  Have not received compensation

Who is not eligible? Hospital patients (in patients, out patients and day patients) Recipients of Commonwealth Aged Care Funding e.g. resident in high care (nursing home) or low care (hostel) facility, CACP, EACH Those eligible for equipment from other programs e.g. DVA, CRS Those who do not have permanent and long term disabilities e.g. an individual recovering from surgery

Getting equipment through CAEP 1.GP completes CAEP referral form. 2.Individual takes form to service provider and receives assessment of clinical need. 3.Specifier recommends equipment to meet the basic and essential need. 4.Equipment is ordered, specifier makes sure that the item is appropriate and demonstrates correct and safe usage.

CAEP equipment is for use within the home. What is the definition of home? CAEP defines the home environment as the person’s primary residence and includes external access to the clothesline, letterbox and garage. The CAEP Clinical Sub-committee will consider individual cases for full access to the home’s external environment when that environment is a farm or a few acres of land.

Who owns the equipment? All equipment is provided on a long-term loan basis An individual can use the item for as long as there is a need for it When there is no longer a need, the equipment must be returned to the CAEP Service Provider so it can be re-issued

Who pays for equipment repair? It is the responsibility of the individual to keep equipment clean and safe It is the responsibility of the CAEP Service Provider to repair and maintain equipment
