Dreaming was fine – but we didn’t have:  A nation wide platform (infrastructure) that both municipalities and Health Care providers can use for data exchange, regardless of business system used  An national organisation that was willing to be engaged in Remote Monitoring  A documented understanding from the responsible Ministry, of the importance of remote monitoring an integration of care.

Dreaming opened our eyes for an almost existing solution:  A nation wide infrastructure is now rolled out in Sweden – the National Patient Overview Other main purpose from the beginning Will be accessible in a secure way from all relevant staff in: –Primary Care –Specialist Care –Municipalities

To take care of different alarms  Emergency alarm Same organisation as taking care off 112 calls  All other alarms New organisations where adjacent municipalities cooperate to achieve large scale economics

And we also need:  Integration between the nation wide platform and “business system”. No log in to other systems  EHR:s should be possible to show monitored data in “trend format”  Integration to the remote monitoring equipment  A safe “Bank Vault” for storing the monitored data Make sense of concept “Personal Health Record”?
