Coal Mine Serious Accident OVERVIEW On Friday, September 19, 2011, a 32 year old roof bolter operator, with approximately 14 years of mining experience, was seriously injured while waiting for the continuous miner to complete a cut in the No. 1 entry. The victim was positioned inby the corner of the last open crosscut in the No. 3 entry when a rock (kettle bottom) fell from between the rows of bolts and struck him. The victim had heard the mine roof work overhead while sitting on his knees. He was struck by the kettle bottom while attempting to stand. The roof bolter operator’s leg was amputated as a result of his injuries.

He was struck by the fallen material while attempting to stand. An indention was left in the mine roof where the kettle bottom was located.

BEST PRACTICES 1.Conduct a thorough visual examination of the roof, face and ribs immediately before work or travel is conducted in the area; thereafter as often as conditions warrant. 2.Miners shall be alert to changing roof conditions and proper corrective actions shall be taken to control roof abnormalities.
