Critical Task Based Job Hazard Analysis Example Step 1: As part of the Walkdown Start with the major work activities and list them as tasks in the JHA template: 1.Decon outside area- path to C Decon inside tent 3.Airline recovery-investigation This is at too high a level to ensure that everyone has the same mental picture of the tasks for the identification of hazards. Step 2: Still as part of the walkdown Identify tasks in more detail as subtasks to the initial task: Decon outside area- path to C-202 Survey and determine extent( including top of scaffolding) Check Scaffolding inspection completed Remove covers/ plastic covers Scoop up soil Wipe down and apply fixative to component Decon inside tent Extend CA outside for ease of suit/unsuiting Remove and/or dispose contaminated items- plywood, bottle rack outside Soil contamination outside decontamination Survey and determine extent of contamination Survey media for labs, determine dose rates on bagged Clothing (40Rad) Wipe down and remove flooring Survey under removed flooring, decon if required Replace flooring Wipe down benches, tables, tent Airline recovery-investigation (assume pressurized) Install Glove bags Lock and tag Release pressure on Airline. Packaging of Air Line Move airline to AV1. Tasks Sub Tasks Step 4: Once the critical tasks are identified then the hazards can be identified and error likely situations can be discussed: Release Pressure on Air Line Contamination spread Require Work steps to release pressure Installation of Containment large enough to contain any residual pressure in hose. Hold Points for Containment installation and removal Critical -Tasks Critical Task Hazards Step 5: Once the Critical Task hazards are identified use the worksite hazard analysis and Job Safety Plan to identify controls and methods of implementation Contamination spread Require Work steps to release pressure Installation of Containment large enough to contain any residual pressure in hose. Hold Points for Containment installation and removal Possible mistakes during the critical tasks include: Puncture of glove bag with drill bit. Mitigate by ensuring that the drill bit is covered with absorbent material when not actually in use. (Precautions) Worst thing that may go wrong: Hose depressurization blows off glove bag. Ensure workers are in full anti- contamination clothing with full face respirators or SCBA during the depressurization. Ensure unnecessary personnel are not near the work area. (Warning prior to steps) Non-routine Activity: This is a first time activity, a thorough briefing of the work should be performed prior to performing the work. (Precautions) The four methods of implementation listed in the work control procedure. 1 - Controls within the qualification or training of the worker that are often used do not need to be discussed in the work instructions. 2 - Controls within the qualification or training of the worker but are seldom used and are applicable to the entire work activity should be placed in the precautions as a reminder that the hazard exists and the workers are expected to take the appropriate actions. 3 - Controls within the qualification and training of the workers but are for hazards that are introduced at specific steps or by specific actions during the job should have a warning or caution statement immediately prior to the step but require no detailed instructions to mitigate in the work instructions. 4 - Controls not within the qualification and training of the workers for hazards should have detailed instructions for how the workers are to mitigate the hazard and should be in the work instructions in a way that prevents or mitigates the hazard. REV. 0 Step 3:As part of the JHA meeting and/or development of a Safety Plan Identify the critical tasks: Airline recovery-investigation (assume pressurized) Install Glove bags Lock and tag Release pressure on Airline. Packaging of Air Line Move airline to AV1. Critical Task Critical Tasks are defined as: Steps performed that interface with a structure, system, or component and are irrecoverable or irreversible with potential for direct impact on plant condition or personnel. To be considered a critical task two conditions must be satisfied: The state of the plant, system or component or the well being of the individual depends solely on the individual worker. The outcome of the error is intolerable for personnel safety or the plant (Independent of when the consequences are realized). Critical Task Controls What are the likely mistakes? Is this the way we routinely do this? What is the worst thing that can go wrong while we are doing this?
