PARTICIPATION To participate in AHJS FOOTBALL you will need the following: Pre Participation Physical Evaluation from a doctor on the correct UIL form Can NOT participate in practice without form being turned in 24-hour Code of Leadership Form Sudden Cardiac Arrest Awareness Form Concussion Acknowledgement Form Parent/Student Steroid Agreement Form

EQUIPMENT SCHOOL ISSUED: HELMET, SHOULDER PADS, HIP/THIGH/KNEE PADS, MOUTH PIECE, PRACTICE/GAME JERSEY AND PANTS SUPPLIED BY PARENT/GUARDIAN: CLEATS

HELMET WAIVER

U.I.L. ELIGIBILITY  First 6 weeks will be a grade check.  Report card every 9 weeks.  Progress report every 3 weeks to regain eligibility.  What is passing? 70 or above in all classes.  Waiver for advanced classes (GT or HS credit).  Can only use once a year for one class.
