Documents required General and Specialist practice consultations : MIP members pay the invoice upfront and send to GMC : The detailed original invoices showing the dates of service and the name of the patient, OR GMC MIP – Refund Claim Form dully filled in, stamped by the Attending Doctor and signed by both.
