Claims Process Making a workers compensation claim Worker must have the prescribed medical certificate before being able to complete a claim. A worker should not be advised to complete a claim form.

The worker completes the left hand side – Injured workers details only (Q1-35) The Employer’s details are completed in the IMU (Q36 to 66) A claim is not a valid claim unless a workers compensation claim form is accompanied with a prescribed medical certificate – so do not send one without the other.
