Here are a couple of pictures of what can happen with a snagged stand while running or pulling out of the hole. This recently happened on a Lakota rig. Reviewed this with both crews on Dec 18.

 Stay away when the elevator pick up the drill pipe.  Check the elevator after latch and before driller picks up the pipe.  Inform the driller if you have any problem.  Communication is the key to avoid accidents.  “STOP” the job if you see any thing wrong.
