Scope of Services Workgroup Goal n Determine which services, programs and funding are recommended for integration during Phase I

Scope of Services Workgroup Deliverables n Complete matrix of LTC services currently available in San Diego County n Complete matrix of all LTC services currently not available in San Diego County n Make formal recommendation on services, programs and funding to be integrated in Phase I

Scope of Services Workgroup Accomplishments/Progress to Date n Scope of Services Workgroup established and members recruited n Matrix template has been developed n Information on available LTC service being collected n Meeting regularly since August 1999

Health Plan Workgroup Progress Report Chair: Don Billings

Health Plan Workgroup Goals n Establish linkages with health plans that currently provide primary and acute care services to the LTC population

Health Plan Workgroup Deliverables n Prepare report on barriers to use of home and community-based services by health plan members and efforts to improve identification of and assistance with LTC needs n Provide information regarding the current and proposed health plan activities n Report on “lessons learned” from health plans

Health Plan Workgroup Accomplishments/Progress to Date n Health Plan Workgroup established and members recruited n Meeting regularly since September 1999 n Medicaid and Medicare health plans that serve the LTC population have been identified n Health plan survey and cover letter has been developed to collect information

Health Plan Workgroup Next Steps n Administer health plan survey and compile results n Prepare report on barriers to use of HCBS by health plan members and efforts of health plans to improve identification and assistance with LTC needs n Prepare report with key issues to be addressed for successful integration of acute and LTC services n Ongoing member recruitment and involvement

Governance Workgroup Progress Report Chair: Evalyn Greb

Governance Workgroup Goals n Develop a recommendation for the governance structure for the implementation phase of the LTCIP

Governance Workgroup Deliverables n Prepare report and present information on LTCIP governance options, including experience with other counties/states, legislative authority, potential for risk bearing, relationship with Board of Supervisors n Make formal recommendation on governance structure and rationale

Governance Workgroup Accomplishments/Progress to Date n Governance Workgroup established and members recruited n Meeting regularly since September 1999 n Review and analysis of options for governance structure
