© National Hauora Coalition Service Providers MPT Whānau Ora Practitioners – Proficient in case management competencies – Nursing qualified practitioners with a special interest in families, babies & children – Able to provide immunisation, well child and B4 school checks – Able & skilled to work with whanau to address the social determinants of health including whanau ora planning – May work in teams with community health workers, – Strong focus on parenting and supporting safe families; – Will be part of a Whanau Ora network and able to provide solutions to social issues including housing, economic independence and parenting skills; Lead Maternity Cares – Able to provide antenatal & maternal care with greater support from MPT Whanau Ora Practitioners & in partnership with the medical home able to provide more complex clinical care; Medical Home – All young families are engaged in a medical home which may be concentrated in larger centres; – Additional specialist nursing services for GDM, obesity, mental health including alcohol & drug; – Provides more access for pediatric medical support including observation beds and access to POAC (as described in the GAIHN Child Health Pathway work); – Developed as proactive providers of wellbeing for families; Haputanga/Antenatal Care – Comprehensive antenatal care that is layered with a whanau centred appraoch. Strongly connected to MPT Whanau Ora Practtioner and LMC; – Greater emphasis on whanau, parenting and safe & healthy environments for mothers and babies;
