Sequence of Events AYP-QPA Data Checklist Make sure data is correct EYOA – End of Year Accountability EOYA Collection: End of Year Accountability 1. Submission Window: May 6, 2013 – June 14, Purpose: This record type collects demographic, program participation, and attendance data for use in AYP determinations and other end of year federal reporting. 3. Records for all students (both active and inactive) who attended at any time during the year should be submitted, and data should be current as of the student’s last day of membership for the school year. 4. Focus is on membership, attendance, and truancy information. 5. For information on the required and optional fields for EOYA, please refer to the KIDS Submission Details: EOYA document.

Sign-off before truancy data is checked. Entered data before uploads to the KAN-DIS system were complete Signed off on the AYP/QPA data checklist

KAN-DIS NCLB/IDEA Reset or Un-submit Report Request

KAN-DIS Seclusion and Restraint Unsubmit Report Request Call Laura Jurgensen at (785) Reports may only be unsubmitted while the reporting window is open and must be resubmitted to KSDE before the reporting window closes. The system will unsubmit both the seclusion and restraint reports even if you only need to make changes to one report. Please remember to resubmit both reports to KSDE.

Definition of physical restraint in K.A.R (f) (Use this definition for reporting beginning in the 2013–14 school year.) Restraint can be chemical, mechanical, or physical. Chemical restraint means the use of medication to control a student’s violent physical behavior or restrict a student’s freedom of movement. It is prohibited, except as prescribed treatments for a student’s medical or psychiatric condition by a person appropriately licensed to issue these treatments. Mechanical restraint means any device or object used to limit a student’s movement. It is prohibited unless ordered by a person appropriately licensed to issue the order for the device, for a specific student. Physical restraint means bodily force used to substantially limit a student’s movement.

Definition of Seclusion in K.A.R (h) (Use this definition for reporting beginning in the 2013–14 school year.) Seclusion is not a place, it is a process. Seclusion is identified by meeting three criteria: The student is… (1) Placed in an enclosed area by school personnel. (2) Purposefully isolated from adults and peers. (3) Prevented from leaving, or reasonably believes that the student will be prevented from leaving, the enclosed area.
