Provide a copy of AFI to the AD/CC and ensure he/she is aware of what to do **The key is to know each role and be ready to provide guidance to each POC that may need assistance. Follow-up is a must! 4

I n t e g r i t y - S e r v i c e - E x c e l l e n c e AD/CC Responsibilities The Commander should… Ensure that the MPF submits the initial Death Report (see Attachment 35), within 24 hours of learning of the member’s death Make every effort to notify the NOK in person, if they are not aware of the member's death Help the NOK apply for benefits Ensure the MPF prepares and distributes documentation (see paragraphs on deaths from injuries or diseases incurred or aggravated while the member was in a duty status) 6

I n t e g r i t y - S e r v i c e - E x c e l l e n c e AD/CC Responsibilities Casualty Reporting The commander of the member’s attached unit forwards appropriate documents to HQ ARPC/DPSPE and provides casualty assistance If all the necessary info about the death isn’t available, the member’s commander asks the Air Force unit or installation nearest the place of death to verify the death and furnish all relevant data Units and installations receiving requests for info must send it to the commander as soon as possible ** It’s important for BIMAAs to assist in this area – Lots of follow-up 7

I n t e g r i t y - S e r v i c e - E x c e l l e n c e AD/CC Responsibilities When the Air Force learns of an USAFR member's death from any source other than the NOK, the member's commander should confirm the death before contacting the NOK If the NOK are unaware of the death and they reside near the member's unit of assignment, the member's commander should notify them and provide circumstances and condolences in person if at all possible The member's commander should contact the NOK within 24 hours of learning of the death to arrange for casualty assistance 8

I n t e g r i t y - S e r v i c e - E x c e l l e n c e CAR Responsibilities Casualty Assistance HQ ARPC/DPSPE should… Contact the NOK for IMAs, participating Individual Ready Reserve, and inactive Reserve personnel The CAR should … Telephone the NOK to help them apply for benefits Advise the NOK that the Air Force can provide further assistance through the mail or in person at the MPF Verify the NOK's address Complete and submit applications for monetary benefits and other privileges Resolve any problems in settling the deceased's estate and any complaints from the NOK 9

Pleasant Ave., Livingston NJ Refer inquiries regarding SGLI to HQ ARPC/DPSPE, 6760 East Irvington Place #4000, Denver CO (DSN or , extension 71227) Refer inquiries about RCSBP to HQ ARPC/DPSPE, 6760 East Irvington Place #4000, Denver CO (DSN or , extension 71288) 10

I n t e g r i t y - S e r v i c e - E x c e l l e n c e CAR Responsibilities Service, Veterans Affairs, and Social Security Benefits The CAR should… Inform the NOK of benefits they might be entitled to claim Advise the NOK to contact the Veterans Affairs Regional Office and the Social Security Administration nearest their residence for further information Advise the NOK to contact their local and state office or department of veterans’ services or affairs for information on any additional benefits that might accrue 11

I n t e g r i t y - S e r v i c e - E x c e l l e n c e HQ ARPC Responsibilities Determining Eligibility for Benefits HQ ARPC/DPSPE determines whether the NOK of ANG and USAFR members who die in a non-duty status may receive certain benefits Service members’ Group Life Insurance (SGLI) HQ ARPC/DPSPE certifies a deceased member's SGLI coverage The Office of Servicemembers’ Group Life Insurance (OSGLI) determines each claimant's eligibility to be an SGLI beneficiary If the UPRG is available and contains a VA Form SGLV 8286, the servicing MPF gives each potential beneficiary a VA Form SGLV 8283 after forwarding the required casualty package to HQ ARPC/DPSPE (see Table 6.1.) 12
