Part One- How/What/Why (Description) Succinct Description: Partners/Sites Member Types/Terms Activities Outcomes Most Important Facts That Would Interest/Inform Everyone

Part Two- Your Team Team Roster Everyone that has a role in the success of this program should be a part of your team Management Teams need to stay in contact, meet regularly, assess progress

Part Three-Communication and Oversight Keeping Stakeholders Informed Management Processes Training Assessment Processes Financial Oversight/Request Process Clearly Stated Expectations for Easy- to-Follow Program Processes

Part Four- Preparation Spend Some Time Before Year Starts Talking About Potential Problems Are files in different places? Is there a really busy partner with a role? Think of the one thing that keeps you up at night

Part Five- Comprehensive Timeline Each Program Year Task from Beginning to End Who is Responsible? Who will help? Due Date? Actual Date? If items aren’t finished on time, revise and remind

Part Six- Execute Final Document Signed by ED, PD, and PO Changes/Revisions require new signature page
