How respite will be accessed under the Commonwealth Home Support Programme Access to Commonwealth Home Support Programme services will be facilitated through screening and assessment undertaken by My Aged Care and Regional Assessment Services. It will identify: -The care needs of the care recipient -Type of care provided by the carer -Any difficulties/concerns with the caring arrangement -Sustainability of the caring arrangement -Any supports that may assist with the caring role Assessment will result in the development of a support plan, based on client goals and agreed actions to be taken, such as referral to respite and other CHSP services. l National Respite Association Conference - October
