13-May-15Saskatchewan Falls Collaborative3 Aim Aim: To reduce the number of falls by 20%, reduce injuries by 30% by the end of March 2012 Goals: To fully implement a Fall Prevention Strategy at Parkridge Centre as mandated by the Ministry of Health

Aim Improvement Objectives: All staff will receive education on fall prevention and the new Fall Prevention Strategy in Sept 2011 All new admissions, including respite clients will have a Scott Fall Risk Screen completed on admission All new admissions who score 7 or greater will have a Care Guide for Fall Prevention completed All current residents will have a Scott Fall Risk Screen completed by Nov 30, 2011 All resident rooms will have a Falls Safety Assessment completed in Oct 2011
