Registration Process for New Online Certificate Entry

Option 1 – Click on PwD Registration Step 1 – Open website Step 2 – Click on Registration Tab Step 3 – Click on option 3 for online PwD Registration Step 4 – Fill up all fields in form appropriately - (fields in red color arewww.ability.gujarat.gov.in mandatory) Step 5 – Click on Submit button Step 6 – Export report in PDF format and Save it in your computer

Registration Process for Old Certificate Entry

Steps for Old PwD Registration Process Flow Enter all mandatory fields marked as Red color in registration page. Old Certificate Entry : All the other process is Same as New Registration - Change in Past Details as stated Below Steps Select option “Did you ever apply for issue of a disability certificate in past? :” – YES Select Facility name from “Authority to whom and District in which Applied” and “Result of Application”. Select type of disability and Certificate expiry period in month and year for particular disability (Please do not select Disability Details Applying For) Upload scan copy of Old Certificate from Upload Certificate option (Optional).

Option 2 – Click on (Download physical survey form English / Gujarati (Form-1)) Step 1 – Click on Registration tab in the website Step 2 – Click on Download Physical Survey Form in English / Gujarati(Form-1) Step 3 – Save downloaded form in your Computer Step 4 – Generate Print of Form 1 and fill the form manually Step 5 – Post the filled Form 1 to Disability Cell for PwD Registration Step 6 – You will receive a call from Disability Cell once your PwD ID is generated
