Learning Disability Self Assessment Framework – easy read summary Hertfordshire 2014

What’s the big idea? In the past we did this in two ways. These were the: Health Self-Assessment Framework The Learning Disability Partnership Board Report This is the second year of the Learning Disability Self Assessment (LD SAF)

What we checked? The big things (sometimes called measures) people said we needed to ask questions about were: Staying Healthy- This includes lots of things such as getting a good service from the doctors, chemist, dentists or when going to hospital Being Safe- This includes lots of things such as in their own home, in hospital or out and about where they live Living Well- This is about lots of things such as having a say about how services should would or being welcomed by their community

How did we check these ‘measures’ Compliance - We asked for evidence from different commissioners, providers, teams, and services both health and social care, self advocates and family carers Data- we were asked about numbers

What we found out in the data part There are still areas where we couldn’t find the information we needed. We are now looking at how we can get this information, so we can be better at checking next time.

How we rated (scored) in the self assessment part Each measure was rated (scored) as red, amber or green Staying Healthy rated: red - 2 amber - 4 green – 1 Being Safe rated: red - 1 amber - 7 green - 0 Living Well rated: red - 0 amber - 7 green - 1

What will happen next? IHal will give Hertfordshire an overall rating We will be taking the LD SAF to the Health and Wellbeing Board in March We will be putting together an Improvement Plan – to help make things better in Hertfordshire: To make things better where we rated them red To Look at how we can make things even better where we have rated them amber
