Crianlarich – Potential High Impact Near Miss

Potential Injury at Crianlarich Background When the door of the Bass Wagon was opened several Steel Sleepers fell to the ground. There was the potential for the sleepers to strike the people on the ground and cause serious injury The bundle of sleepers was Banded at source and were secure when received by Babcock. The Banding failed during the train’s transit from Kingmoor to CrIanlarich. It is likely that the bundle would have been lifted and moved numerous times prior to being received by Babcock which may have weakened the Banding.

Potential Injury at Crianlarich Lessons Where practical visually check the load prior to opening the door. Use the RRV to control the door lowering when opened. Maintain Exclusion Zones. Working with moving equipment It is apparent that an effective exclusion zone was implemented and maintained while opening the Wagon door. This prevented any person being struck by the falling sleepers and potential significant injury.
