Plan For Routine EPI Outreach vaccination

Situation Analysis Establishment of objectives Assessment of resources/Fixing priorities Develop and Implement action plan Monitoring and Evaluation PLANNING CYCLE

 Allot geographical area (Ward/U.C) to each vaccinator accordingly  If more than one vaccinator in a UC/Ward then, divide the area according to the population  Divide the area equally, establish vaccination site in each area, acceptable to community (parents)  Preference should be given to Health House as Vaccination site, LHWs will be helpful in micro census

 Supervisors will be responsible for logistic provision to the LHW (s) in EPI uncovered areas.  Specify vaccination day of the month for each site  Ensure availability of U.C/Ward Map with target population at every vaccination centre  Check vaccine carrier before going in the field  Before starting vaccination, make arrival announcement in the area

-Social mobilization in the newly populated areas populated areas -Selection of vaccination site acceptable to community -Specify vaccination day of the month -Nomination of Volunteer Focal Person (preferably LHW )

 Wash your hands  Appreciate the mother for bringing his child to vaccination site  Registration of newly born baby  Record keeping of vaccination  Inter vaccination date, month and year in vaccination card and register  Inter name, father name, age, and address of child  Date of next visit

 Tell mothers that which vaccine has been administered  When mothers should re-visit vaccination site  Inform about side effect of vaccine, if so what to do?  Keep the card safe and bring along with on next visit  Inform mother, that bring the child for vaccination on due date irrespective of child health status

 Thank all those who help out you  Collect garbage and clean vaccination site  Collect used vials etc and put it in a plastic bag  Put all used syringes in safety box  Put leftover vaccine in vaccine carrier except BCG and Measles

 Shift the left over vaccine to Refrigerator/ILR  Mark the leftover vaccine and keep it in return box
