Ohio Fire Incident Reporting System Welcome to OFIRS Ohio Fire Incident Reporting System

General tips for ALL Reports Don’t forget to add occupants & owners names

$ Dollar Loss is an estimate $ There are several methods of calculating Dollar Loss: Ask a contractor the replacement value Contact the Insurance Agent Check with the homeowner County auditor’s website

NARRATIVE IS NECESSARY AND REQUIRED FOR ALL FIRE RELATED INCIDENTS

Reporting All Incidents Helps your department make good decisions about the needs of your community.

Grants Opportunities By reporting to OFIRS you are also reporting to NFIRS (National Fire Incident Reporting System) Several organizations use NFIRS data; NFPA, FEMA, CPSC, ISO, BOCA, EPA Many of these organizations offer grant funds.

Incident Type Categories Fires 111 - 173 Ruptures Explosions 211 - 251 Medical Rescue 311 - 381 Hazard Spills Leaks 411 - 482 Service Calls 511 - 571 Good Intent Controlled Burns 611 - 671 False Alarm Bomb Scare CO calls 711 - 746 Weather Disasters 811 - 815

DO NOT USE INCIDENT TYPE 100 OR 110

THE GOOD NEWS BUT $ LOSS CAN NOT EXCEED $5000 INCIDENT TYPES 113 - 118 ARE CONFINED STRUCTURE FIRES and DO NOT REQUIRE Completion of the FIRE MODULE BUT $ LOSS CAN NOT EXCEED $5000

Fire Incident Types Structure 111 Silos Bridges 112 Confined Cooking 113 Chimney 114 Trash 118 Mobile Home 121 Residence Modular Building 123 Vehicles 131 Motor Home Camper 136 - 137 Mulch 140 Natural Vegetation 141 – 143 Trash 150 151 Dumpster 154 Grills Fence 162 Mailbox 164 Crops & Orchards 171–173

(fire module NOT required) Cooking Fires Contained cooking fire is Incident Type 113 (fire module NOT required) A cooking fire that extends beyond the stove would be Incident Type 111. Area of Origin 24 Equipment Involved in Ignition: Range (646) or Oven (645) Make & model are only important IF the equipment failed to operate properly.

Equipment Involved In Ignition 645 or 646 Check your categories A Cooking Fire Should be Equipment Involved In Ignition 645 or 646 Check your categories Not 124

Chimney Fires A contained chimney fire is Incident Type 114 If the chimney fire spreads beyond the chimney then it would be Incident Type 111. Fire module - Area of Origin 52 Be sure to include the Equipment Involved in Ignition; chimney, fireplace, wood stove, etc. Dollar loss & narrative are important

Incident Type 118 is a contained Trash Fire INSIDE a structure

Incident Type 151 is a Trash Fire OUTSIDE a structure

Outdoor Toilet A favorite target of juvenile firesetters Incident Type 112 Specific Use 926 Area of origin 25 Remember dollar loss $$$ Arson or juvenile?

Dryer Fires Incident Type Code 111 Equipment Involved in Ignition 811 Heat Source 11, 12 or 13

Utility Poles Equipment Involved in Ignition Should be coded Incident Type 162 Even if the fire department does not extinguish the fire, it is still reported as Incident Type 162 Remember to include Equipment Involved in Ignition

Mulch Fires In order to track this type of fire we need to be consistent in coding Incident Type 140 Specific Use 931–965 Area of origin 90-94 Items 1st ignited 44 Type of material 1st ignited 61 Dollar loss

Property/Specific Use 900 Outside series Dumpster Fires Incident Type 154 Property/Specific Use 900 Outside series

Vehicle Fires Incident Type 130 – 138 Action Taken Specific Use ( 900 series ) Vehicle Information

Camper or RV Incident Type 137 Identify the Type of Camper Under Mobile Property Type

??? EXPOSURES ??? An exposure to the original incident is any property of value not connected to the structure of origin. A house fire that spreads to the car in the driveway is a structure fire with the vehicle being exposure 001 A house fire that damages the car in the garage is a structure fire with the car considered contents.

If the fire starts here Vehicle inside the garage is contents

If the fire starts here A vehicle Outside that is damaged becomes an EXPOSURE

ACTION TAKEN At least one Action Taken is required. If you have extinguished a fire, 11 should be at the top of the list You can enter as many actions as you would like but only the top 3 are sent to the Marshal’s office & USFA

Specific Property Use Code Is a required field Vehicle fires do not usually start in the residence (419) but rather in the garage (881) or (882) or in a residential driveway(962) street(961) or parking lot(965) 419 should ONLY be used for residential structure fires

Fire Casualties Many of the national statistics are based on YOUR reports Age is an important factor when developing educational or safety programs HIPAA is not an issue because the information is statistical in nature

Fire Module Number of buildings & type Area of Origin Heat Source Type & form of material 1st ignited Cause of Ignition Factors contributing to Ignition Human factors contributing to Ignition

Is it residential ? What code did you use in Specific Property Use ? How many building are involved ? You must have a number in the residential Living Units OR Check the Not Residential box

These two fields should be describing area of origin is required These two fields should be describing the same item

These 2 fields are now required

Equipment Involved in Ignition is indicated by Heat Source 10, 11, 12 or 13

Equipment Involved or Mobile Property

Choose the Correct Code for Equipment Involved Oven or Range Electrical 200 series 218 Switch 631 Coffee Pot 261 Plug 122 Fireplace 210 wiring 811 Dryer 217 Outlet

Include Brand & Model if the Equipment is defective otherwise just complete the 3 required fields 1 3 A Dryer is not portable 2 Power Source Required

Smoke Detector & Sprinkler information are important

Based on YOUR reports

If you choose the check box “fire spread was confined to object of origin” in the Fire Module then you must check the same box in the Structure Module 1 Fire Spread in the structure module would have to be 1.

Children Playing with heat source is an important statistic In order to track this valuable information you will need to specifically use the following 3 fields: Heat Source = be specific was it a lighter, candle, stove? Factors Contributing to Ignition Codes = 19 Human factors contributing to Ignition = 7 and then list their AGE

ARSON/SUSPICIOUS Cause of Ignition All 3 codes can indicate Arson/Suspicious (1) Intentional (5) Under Investigation (U) Cause undetermined after Investigation Use these 3 codes with care

Mutual Aid Giving Mutual Aid means actually arriving on the scene and participating in the incident If you are receiving Mutual Aid, you include only your apparatus & personnel You MUST list the departments giving or receiving aid to your department If you use Automatic Aid codes 4 or 5, you must list the FDID number of the aid department.

Good Data Correct codes Dollar Loss Fire Casualties Narrative
