ODH Fans and Work Planning Protecting Against the Big Event 10 Questions Before Starting a Job at C-AD Photo of the Week Collider-Accelerator Department Take 5 for Safety

ODH Fans and Work Planning  A safety concern was raised by an F&O electrician regarding the lack of a service switch at two ODH fans at B1008  Electrician was startled by ODH fan not turning off after a fan-motor replacement job and test  The ODH protection system is designed to minimize possible failure points  Local service disconnect switches are a possible failure point  LOTO can be performed at the electrical service disconnect  There are C-AD barriers currently in place to protect workers (required work planning, Orange Tags, contacting ACS Group, procedures, training)  ODH system may not be serviced or modified without C-AD approval as per written agreement with F&O  Proper planning and coordination with C-AD’s Work Planning Coordinator is required every time work is performed by F&O on ODH equipment  F&O’s electrician agreed to clear his LOTO of the system and BNL Chief Electrical Inspector approved the clearing of the LOTO 2

3 Protecting Against the Big Event …poor maintenance of electrical equipment that may have ignited the explosion, bypassing of gas alarms and automatic shutdown systems that could prevent an explosion, lack of training of personnel on when and how to shut down engines and disconnect from the well to avoid a gas explosion, not using a diagnostic tool to test the strength of the cement barrier on the well, ignoring a pressure test that had failed… Conclusion: Need better management of decision making processes within BP and its contractors

Protecting Against the Big Event – 10 Questions to Ask Yourself Before Starting a Job at C-AD 4 1. Are any radiation barriers added, changed or modified by the job or project (for example concrete and steel blocks, earth shields, fencing)? 2. Will the project or job generate new sources of activated or radioactive material? 3. Will the project or job alter or change an interlocked or locked accelerator enclosure? 4. Will the job or project add, modify or otherwise affect an Access Control System? 5. Are the large volume gas systems or their controls at AGS, RHIC, PHENIX, STAR or ERL being worked on? 6. Does the project or job involve altering, enlarging or otherwise changing an ODH area? 7. Are ODH fans or smoke removal fans or intakes in the RHIC tunnel or support buildings being modified in any way? 8. Are the smoke removal fans or intakes in the AGS tunnel being modified in any way? 9. Are environmental systems being worked on (i.e., stacks, filter banks or exhaust fans at BLIP, NSRL and Building 801)? 10. Are ventilation systems in Building 801, BLIP Building 931 or NSRL Building 956 being worked on? If answer is yes, then permission required – contact R. Karol or E. Lessard
