Academician Prof/Dr Hamdy El-kabarity M.D. WINNER OF ERNST THEODOR RIPPMANN MEDAL DE ONOARE Feto-Maternal Immunologic Dialogue Immunologic Hypothesis for the Etiology of EPH Syndrome INTERCONTINENTAL OGASH ACADEMY - CSPP Ain Shams Univeristy Cairo Egypt PRIZEWINNER OF PROFESSOR IOSEB JORDANIA INTERNATIONAL PRIZE-2007

In part I and part II who described the immunologic basis for the evolution of EPH gestosis.1st described for a syndrome of collection of symptoms and signs that occur in: 1. Man kind and high apes 2. Only during pregnancy 3. Once it occurs in 1 st pregnancy, it has tendency to re -occur in the succeeding pregnancy i.e.the 1 st pregnancy may be a test of compatability, incompitabaility of fetus as an allograft.

4. The immunlolgic challenge between the fetus and placenta as an allograft and his hosting mother with the proved imbalance of Th1/Th2 - interleukin 18/12 in normal EPH gestostic pregnancy. 5. The station of vascular endothelial damage we all reached now is not the final station more logically pointig to the changing point to another train. The role of the father DNA as foreign protein to maternal immune systems carried by the deported chorionic villi may lead to the immmunologic imbalance as the respnose to this identification. 6. This immunologic disrupted relation between the mother and the placenta and fetus, this may be to the picture of chorionic endothelial damage that mimic to that appear in the rejected allograft.

Also the antiphospholipid syndrome during pregnancy of abortion, repeated abortion, idiopathic placental insufficiency and toxaemias of pregnancy may be rings of the chain.
