Integration of Health and Care Benefits and Challenges Sarah Pickup Deputy Chief Executive Hertfordshire County Council

The connections Health and Wellbeing Boards Duties around integration Focus on outcomes Organising around patients/people not services Best use of public £s

The Shared Commitment Pursuing a Common Purpose National support for local ambitions Practical tools Integrating information Accelerating learning

What does integration mean? “ I plan my care with people who work together to understand me and my carer(s), allow me control, and bring together services to achieve the outcomes important to me”

The Expectation Integrated Care and Support… (where that adds value by improving outcomes and or reducing costs) Locally driven – one size does not fit all Empowering patients and communities Integrated care and support as the norm in the next 5 years with pioneers moving faster

Spending Review Additional transfer of funds focused on integration ( from 15/16) Counted as if will all arrive in LAs Significant strings – detail to follow Outcomes driver but implicit assumption about efficiency Need to spend on social care may increase further not reduce

In summary High expectations A lot of detailed work to do A real need to work across boundaries – best use of public ( and private) pound Need to focus on prevention and recovery Much we can do The window of opportunity is open!
