A user-friendly, interactive guide for providers to learn about and deliver EBHP A statewide inventory of health promotion programming A county-by-county listing of evidence- based health promotion programs A marketing tool to attract both new partners and new clients to programs A networking tool to identify community partners to implement and deliver health promotion programming

19 What is it, Really? A wiki A website that uses wiki software, allowing easy creation and editing of any number of interlinked Web pages Wikis are often used to power community websites ( accessed 9/21/09)

20 Who Will Find the Roadmap Helpful? Community Providers Resources/strategies to identify partners in the community and to implement EBHP Healthcare Providers Education about what is an EBHP; listings of EBHP and health promotion programming in their community Policy Makers Health, demographic, and health promotion data for each county in the state Researchers

21 Why is this important? Easy access to health information for your community Provides a listing of current health promotion programs so efforts will not be replicated A venue to educate public and providers about health issues in your community, and the importance of EBHP
