Training Objectives 1.Discuss cold chain management (CCM) principles and why CCM is important 2. Identify the storage and handling procedures for ACAM2000 ® 3. Outline the ordering process for ACAM2000 ® 4. Describe safeguards that can be implemented to protect temperature sensitive medical products (TSMPs) 5.Describe the destruction/disposal process for ACAM2000 ® 6. Define Executive Summary (EXSUM) Procedures By the end of this presentation, you should be able to:

What is Cold Chain Management? Cold Chain Management 2-8ºC Preparing TSMPs for shipment to ensure temperatures are maintained during the entire shipping process. Assures vaccine safety and efficacy Saves tax payers dollars Prevents vaccine compromise Assures maximum shelf life Minimizes rate of deterioration

Partnership with Commercial Carrier Guarantees Express Delivery and asset visibility until delivery is completed Use of Validated Shipping Containers to keep products chilled at proper temperature, yet prevent FREEZING Inclusion of Temperature Monitoring Devices Involvement with knowledgeable Customers Key Principles of Cold Chain Management/Distribution Cold Chain Management distribution is a combination of Product Packaging Logistics Mechanical hazards Variable environmental conditions RISK MANAGEMENT Cold Chain Management distribution is a combination of Product Packaging Logistics Mechanical hazards Variable environmental conditions RISK MANAGEMENT Objective 1: Discuss cold chain management principles and why CCM is important Objective 1: Discuss cold chain management principles and why CCM is important

Storage and Handling Procedures (Vaccine) All un-reconstituted ACAM2000 ® will have an 18 month expiration date Un-reconstituted ACAM2000 ® vaccine will be distributed and stored at 2-8 º C (36-46 º F) Un-reconstituted ACAM2000 ® vaccine should not be exposed to temperature conditions º C (73-81 º F) for more than 48 hours Un-reconstituted ACAM2000 ® Objective 2: Identify storage and handling procedures for ACAM2000 ® Objective 2: Identify storage and handling procedures for ACAM2000 ® STRATEGIC NATIONAL STOCKPILE USE ONLY

Storage and Handling Procedures (Diluent) During shipment diluent can be shipped between 2-30 º C (36-86 º F) The diluent for ACAM2000 ® vaccine must be stored at room temperatures º C (59-86 º F) The expiration dating on the diluent is 5 years from the date of manufacture Diluent for ACAM2000 ® Objective 2: Identify storage and handling procedures for ACAM2000 ® Objective 2: Identify storage and handling procedures for ACAM2000 ® REMEMBER: RECONSTITUTE WITH ONLY 0.3ml OF DILUENT

VIDEO Objective 3: Outline the ordering process for ACAM2000 ® Objective 3: Outline the ordering process for ACAM2000 ® Shipping and Handling Video

Ordering Procedures New Customers MM ust pre-register on line to order smallpox vaccine SS elect smallpox vaccine order form UU SAMMA DOC personnel and local Service POCs validate registration  D D OC enables customer in DOC’s secure server CC ustomer receives authorization to access DOC’s secure webpage via Objective 3: Outline the ordering process for ACAM2000 ® Objective 3: Outline the ordering process for ACAM2000 ® Previously registered customers for smallpox or anthrax vaccine  No need to re-register  Go to select vaccines & temperature sensitive medical products, and then select smallpox vaccination programhttp://  Use the same user name and password previously assigned

Strategic National Stockpile and National Stock Number Strategic National Stockpile  Packs and ships ACAM2000 ®  Partnered with commercial carriers for shipping CONUS shipments on Mondays thru Wednesdays OCONUS shipments on Fridays Emergency shipments are handled on a case by case basis NSN: , Smallpox (Vaccinia) Vaccine, Live with diluent, syringes, and needles

Safeguarding of Temperature Sensitive Products Alarms and Backup Generators Assign Duties to Personnel who can:  Monitor operation of the vaccine storage equipment  Set up and maintain a monitoring/notification system  Assure appropriate handling of vaccine during a disaster, power outage or mechanical failure Establish resource contact lists  Name, title, home phone, pager, cell phone of the primary and backup  Equipment repair  Packing containers, gel packs  Backup storage  Plan to transport product to backup site Objective 4: Describe safeguards that can be implemented to protect temperature sensitive medical products (TSMPs) Objective 4: Describe safeguards that can be implemented to protect temperature sensitive medical products (TSMPs) ELIMINATE LOSS

Vaccine Disposal DoD activities are responsible for disposal of compromised or expired vaccine ACAM2000 ® vaccine can be disposed:  In a hospital sharp container  Processed for destruction using a returns program Destruction memorandum should be routed up the chain of command prior to submitting to the US Army Medical Materiel Agency (USAMMA) Distribution Operations Center (DOC) Disposal instructions can be found at: Destruction Codes and be found on the USACHPPM website: Objective 5: Describe the destruction/disposal process for ACAM2000 ® Objective 5: Describe the destruction/disposal process for ACAM2000 ®

Executive Summary (EXSUM) Must report suspected loss of ACAM2000 ® that occurs for reasons other than expiration within 24 hours Executive Summary (EXSUM) submitted in Memorandum Format  No longer than one page in length  Explain circumstances surrounding suspected loss of vaccine potency  List Lot numbers  Number of unopened vials  Detailed explanation of course of action to preclude future losses  List names and telephone numbers of points of contact  Fax to USAMMA/DOC pharmacy consultant  More information available at: Objective 6: Define Executive Summary Procedures Objective 6: Define Executive Summary Procedures

Cold Chain Management (Equipment Requirements) Choose the appropriate thermometer  Use a certified calibrated thermometer to assure validity  A certified, calibrated thermometer in a biosafe liquid is an inexpensive, adequate “quick read” monitor  Constant read/graph thermometers continuously record temperatures either on paper or electronically  Digital thermometer/Alarm units are the most accurate constant monitors and also offer continuous alarm capability Thermometer location  Place thermometer in a central location within the freezer such as the middle shelf  Place on the top shelf only if the refrigerator compartment is not in the path of the cold air vent from the freezer  Never place thermometer in the door of a refrigerator compartment ACCURACY CERTIFIED CALIBRATED THERMOMETER = VALIDITY & RELIABILITY Objective 4: Describe safeguards that can be implemented to protect temperature sensitive medical products (TSMPs) Objective 4: Describe safeguards that can be implemented to protect temperature sensitive medical products (TSMPs)

Actions Required to Prevent Vaccine Compromise/Loss The following actions help prevent vaccine compromise  Reading thermometers and recording temperatures at least twice a day  Assign a log manager and a back-up log manager  Taking action when the thermometer is out of range  Not opening and closing refrigerator during loss of power  Filing the completed temperature log at the end of each period  Ensuring all completed temperature logs are kept for at least three years COMMON MISTAKES Failing to read the thermometer and record the temperature twice daily Reading the thermometer incorrectly Documenting out of range temperatures without taking action
