Agenda and Meeting Objectives  Discuss feedback on draft CQI plan and determine changes to be made  Strengthening the “Culture of Quality” and what we can learn from “The Seven Wastes”  Provide update on HV CoIIN and other CQI activities  Determine month and location for face-to-face meeting  Summarize next steps

Draft CQI Plan Feedback Eleven members provided feedback prior to the meeting.  5 of 11 indicated their approval of the plan as is  6 of the 11 provided suggestions or asked questions that indicate further explanation could be useful.

Draft CQI Plan Feedback Suggestions: Add a background section to include information on:  the federal MIECHV program (goals, accountability, data and outcomes for children, and the rationale for CQI and purpose/expectations for the plan)  the three home visiting models, eligibility requirements, target population (also add model developers to the diagram in Appendix 4)  the purpose of the CQI plan; make sure the connection to the data collection and benchmark outcomes is clear (add hyperlinks to the other plans)

Draft CQI Plan Feedback Suggestions: On page 7 (CQI Team Roles), add the following:  Under State CQI Team responsibilities, add “Development and dissemination of driver diagrams” and “Provision of technical assistance [on CQI projects]”; Add facilitation of learning collaboratives around specific content areas?  Is there redundancy in this section regarding stakeholder involvement and local participation? On pages 8-9 (Implementation of CQI), distinguish three phases of implementation:  Creating a Culture of Quality (training, team formation)  Ensuring data is accurate and complete (opportunities for CQI)  PDSAs focusing on family engagement and retention, breastfeeding, developmental surveillance and screening; After baseline data is available for the benchmarks, identify additional areas.

Draft CQI Plan Feedback Suggestions:  On page 9, clarify language around Year 1 data providing a baseline and prioritizing CQI projects around areas in need of improvement.  How do we know what CQI activities are happening in the 11 sites and what they need? Quarterly update as a deliverable, such as the one created by Utah?

Draft CQI Plan Feedback Suggestions:  On page 9, clarify language around Year 1 data providing a baseline and prioritizing CQI projects around areas in need of improvement.  On page 10, Figure 3 is “more whimsical” in comparison to the rest of the document. Options are to find a more formal description of “The Seven Wastes” or to go in the opposite direction and add more graphics that are less formal. Who is the primary audience and what would appeal most to them?  General comment; If incorporated, determine the best location(s): “It is very comprehensive in term of all factors related to effective implementation of CQI. The one issue I would raise concerns the methodology related to model fidelity… I was thinking there could be more information related to assessing fidelity by program. Are there fidelity specific reports/reviews/assessments required of these programs that are currently mandated by the national offices of each program? I would be interested in knowing that. I think it's possible to deliver quality services, but if they don't meet the model specifications unique to each program we may end up with specious data.”
