eclipse A new era in medicines management

What is The Purpose of Medicines Management? – Educating Prescribers on Optimal Prescribing. – Improving Cost-effectiveness of Prescribing – Improving the Quality of Prescribing. – Improving Safety of Prescribing – Reducing Hospital Admissions – Saving Lives

What is the Traditional Model? – Intelligent dedicated prescribing advisors. – Good Advice on a limited number of prescribing projects. – The same goals to all surgeries irrespective of how well or badly they are doing. – Detailed analysis on how well these few projects have done with little analysis on prescribing outside these areas. – Projects set up annually with limited ability to change to market fluctuations. – Very little analysis on the impact on admissions, referrals, QOF and patient outcome. – Difficulty in assessing true benefit for the NHS.

What Model Do we need in the new era? – Educate prescribers based on their own prescribing anomalies FormularyCostInteractionsOutcomes – Give bespoke prescribing projects to each surgery. based on their own potential benefits. – Measure the actual overall cost-effectiveness of prescribing. Drug Spend Patient outcomes – Monitor the quality and appropriateness of every single prescription. – Measure the impact of improvements in safety. Patients admissions. Prevalence of interactions. – Detailed analysis on collating prescribing patterns with outcome data on. Patient Admissions, referrals. Clinical Performance. Patient outcomes. – Analyse the prescription profile of every patient. – Measure exactly how much we save the NHS through our actions. – To generate a revenue stream for the NHS if possible.

What is the purpose of eclipse? – To allow complete monitoring of prescribing. – To allow individualised projects to be automated for each surgery. – To track savings being made from alterations. – To combine ePACT, QOF, Admissions data – To allow every patient’s prescription to be analysed in detail.

Why Does It Work? – Traditional model is where the majority of time in medicines management spent on steps 1-4. – Eclipse will do this for you allowing your team to concentrate on implementation.

STEP 1 - Identification of Audit Topic – Switches – Projects – Safety issues – Price Changes – Rebates TEAMWORK!

ECLIPSE allows each team to set up their own switches / projects. Other PCTs will automatically see the benefit they would get from implementing the same projects. Impact of Projects and switches is calculated at individual surgery level to enable bespoke prescribing schemes for each practice.

Step 2 – Set Standard Need to Know – Where we are now – Where we are relative to SHA and National – Potential Savings from reaching proposed %

Step 3 & 4 – Collect & Analyse Data Single ePACT upload each month automatically populates all projects, switches and formulary analysis at national, SHA, PCT, PBC and surgery level.

Step 5 - Implement League tables of absolute surgery performance. League tables of flux in surgery performance. – Formulary – Switches – Projects – Admissions

Step 6 – Evaluate Impact How much have you saved. What is the impact on performance. What is your team worth?

Evidence is vital. Statins in Norfolk – On paper we lost £27k this financial year – Actually saved £413k cf national.

Eclipse Live! Draws uploads from GP systems overnight Stores anonymised data on eclipse Allows medicines management to get live prescribing data. Allows feedback on dangerous interactions Patient Safety.
